    I watched a safety manager confidently hand an OSHA inspector his emergency response plan. 30 minutes later, his face turned white. The inspector found 3 critical failures that led to a five-figure citation. After reviewing many emergency response plans across manufacturing and pharmaceutical sites, I've discovered these same 3 issues appear repeatedly: 1. The "once and done" mindset ↳ Plans created years ago sitting untouched ↳ Contact lists with people who left the company ↳ Equipment procedures that don't match current machinery ↳ Evacuation routes that no longer exist due to facility changes OSHA 1910.38(f) requires reviewing your plan when: - Employee responsibilities change - The plan is developed - The plan is changed Fix: Set a quarterly reminder to review your plan. Document each review with date and signature, even if no changes were made. 2. Training that exists only on paper ↳ Employees sign training logs but can't explain procedures ↳ Everyone knows to evacuate but not where to assemble ↳ Shift workers who missed the annual training altogether ↳ No practice drills in the past year OSHA expects employees to demonstrate knowledge of their roles, not just show paperwork. Fix: Run scenario-based drills quarterly. Ask employees: "What would you do if..." and document their responses to identify gaps. 3. Site-specific hazards ignored ↳ No procedures for specialized equipment emergencies ↳ Missing response plans for chemicals actually present on site ↳ Generic templates that don't address your unique operations ↳ Failure to address temporary work zones and changing conditions The most expensive citations come from plans that fail to address the hazards specific to your workplace. Fix: Walk your facility with fresh eyes. For each area, ask "What's the worst that could happen here?" Then ensure your plan addresses it. Strong safety leadership means recognizing that emergency preparedness isn't just compliance. It's protecting your team when they need it most.     How Can Write Emergency Procedures? The establishment of sound practices in LPG storage operations to forestall the development of risk situations. Each of the subsections on product transfer, water drawoff, sampling, and venting of noncondensables covers procedures that can lead to unwanted LPG releases to the atmosphere if improperly performed. Although avoidance of such risks is of prime importance, emergency situations may still occur. Effective pre-emergency planning and the use of written emergency procedures at each location for each potential emergency can significantly reduce any adverse impact on plant personnel and neighboring properties. Pre-emergency planning enables thoughtful analysis of each contingency under unstressed conditions and helps to assess the adequacy of the existing organization, personnel, and equipment. Testing a plan using training exercise or drills is an important part of pre-emergency planning. A major concern associated with LPG storage is accidental LPG release to the atmosphere. Whether or not these releases are ignited, the principle considerations in emergency planning must be as follows: a. What steps should be taken to terminate the release? b. What should be done to disperse a vapor cloud to minimize the exposure to personnel, facilities, or the community?  Some of the contingencies that might be addressed in the written emergency procedures: 1-) Fire(Jet fire, UVCE, BLEVE). 2-)Overfilling: Are high-level instruments installed to warn of an imminent overfill condition? Can flow be shut off, or will rapid closing of valves cause surge or overpressure elsewhere in the transfer system? Are adequate shutdown and isolation valves available to rapidly stop or divert flow? What is necessary to avoid overpressure when flow is stopped? 3-)Overpressure: Is high pressure caused by accumulation of noncondensables? Can they be safely vented? Could high pressure be temporarily reduced by pump-out from the vessel or by cooling the shell of the vessel with water spray? Is depressurizing by venting possible? Are unit rundown temperatures too high, causing the LPG vapor pressure to be too close to the pressure relief valves set pressure? Has cross-contamination occurred with a higher vapor pressure liquid? 4-)Pressure relief valve leakage: If high vessel pressure is the cause of pressure relief valve leakage. If tank is less than 90 percent of pressure relief valve set pressure, can the block valve under the relief valve be closed and immediately reopened to see if the relief valve closes(change-over valve)? 5-)Flange leak or piping failure: Is a valve available to isolate the leak?     Every security station, whether in a corporate organization, government institution, residential estate, industrial facility, or commercial premises, should have well-documented Standard Operating Procedures Without written SOPs, security personnel are likely to respond differently to similar incidents, leading to confusion, delayed responses, poor decision-making, and avoidable mistakes. A security operation that relies solely on verbal instructions or assumptions exposes both the organization and its personnel to unnecessary risks. Well-written SOPs eliminate guesswork by clearly outlining what should be done, when it should be done, how it should be done, and who is responsible. As security professionals, whenever we are deployed to a new station, our first responsibility should be to familiarize ourselves with the station's Standard Operating Procedures.     Emergency and crisis management entails preparing for and responding to critical situations that could disrupt normal operations or pose significant risks to an organization. Under the security risk management (SRM) framework policy, organizations should aim to incorporate emergency and crisis management goals thus getting more value from the security function in building resilience and business continuity. Emergency and crisis management protocols that should be incorporated within the SRM program include :- 1. Emergency threat identification – This should be an initial element as the organization’s emergency response will be determined by the nature of threat and personnel capability to identify it. 2. Emergency notification plan - In the event of a critical threat, there should be a communication plan to notify staff as soon as possible about what they are expected to do. This should be managed through tested internal communications channels such as emails etc. 3. Personnel responsibilities - Assign relevant staff respective responsibilities for a smooth emergency response process. A selected group of staff members preferably line manager / supervisors /team leaders should be part of the emergency response management team and should be assigned response actions and are to be notified when an emergency crisis arise. 4. Evacuation and assembly - Include evacuation procedures in case of a threat action that warrants so .The procedures should include proactive evacuation through ideal communications systems as well as reactive evacuation as a result of the discovery or notification of the threat. Evacuation of offsite critical assets and staff should also be factored in where necessary. Assembly areas where a threat warrants external assembly should be factored in. The evacuation plans should include procedures on how to provide safe egress of physically challenged employees to safe zones. 5. Response force and the jurisdiction of organization emergency operational plan - Where the threat has probability of magnitude impact the threat emergency response plan should consist a response force overview outlined in its structure. The design of the organization’s protective system should assume that the response team is capable of handling the threat. 6. Contingency plans – A formal structure responsible for steering the organization through the more strategic implications of a threat. With outward focus towards the stakeholder liaison, establishing contingency facilities, HR issues, maintaining customer confidence, maintaining financial well being, coordinating and maintaining communication with stakeholders etc   7. Business continuity – Establish recovery time objectives for various aspects of business operation, offsite recovery contingency capacity operating at both the strategic level and at departmental recovery level with individual responsibilities under the plan cascaded down the organization. #Securityriskmanagement
